Due to high demand and limited clinician availability, we are not accepting referrals for minors with Oregon Health Plan at this time. Please check this page for updates frequently.
We are accepting patients by referral only for:
- New Patient ADHD Assessment and Evaluation
- Group Therapy
Providers: If you would like your patient added to our wait list, please submit, fax, or mail the referral below to our intake coordinator. Depending on the complexity of your patient’s needs, we may refer them to other providers in the community. If they have been diagnosed with a psychotic or personality disorder, dissociative identity disorder, or require substantial support with autism – we are not equipped to meet their needs. If they have an active substance abuse disorder and have not been clean and sober for six months then we require your patient to first seek treatment. We are not equipped to meet the needs of patients who are registered sex offenders. If your patient faces any of these unique challenges suggestions for care can be found at the bottom of this page.
Once the referral is received, the patient will be added to the waitlist. Please refer to the wait times above for an estimate of time before the patient will receive a call to proceed with scheduling their first appointment.
We are not an urgent, crisis, or emergency facility. If you are having a mental health crisis or emergency please go to the emergency room, contact your county crisis line, or dial 988 for national crisis assistance. Do not communicate urgent, crisis, or emergency needs or concerns via the following form.

We are now accepting direct referrals for certain groups held at Northwest ADHD Treatment Center. Providers should use the form below to refer their patient directly for group therapy.
